Information for Patients

Appointments to See Doctor or Nurse

Consultations with the Doctors are by appointment only. You can choose which Doctor you prefer to see, but we recommend you consult the same Doctor throughout the course of any one illness. If you need to be seen urgently and don't have an appointment, try to phone us before 10.30am so that our Receptionist can arrange for you to be seen as soon as possible.

If you're unable to keep the appointment, let us know as soon as possible. If you don't attend for your appointment, you'll be sent a letter for each appointment you've missed. Fail to keep three appointments, and you will be asked to register with another Practice in the area.

Home Visits

If you're too ill to attend the Surgery, we can visit you at home. Please remember, the Doctor can see four or five patients at the Surgery to every one that he sees at home. Make sure that your request for a visit is made as early as possible, preferably before 10.30am. If the visit is very urgent, please let the Receptionist know.

Suggestions and Complaints

We welcome any suggestions you may have for improving the services we offer. These can either be directed to the Doctors, or via the Practice Manager.

This Practice operates a Complaints Procedure. If you wish to make a complaint verbally, or in writing, please contact the Practice Manager. Alternatively, you can contact the Complaints Manager at Caerphilly Local Health Board by phoning 01443 862056.

Other Information

You can ask to see and request copies of your medical records (we may charge for copies). Requests from Insurance Companies or Solicitors need to be made in writing with your approval.

Any Patient demonstrating violent or abusive behaviour will not be tolerated and reported to the police, and may be removed from our list with immediate effect.
